New Patient Registration

If you live within our practice area and would like to register as a patient, please come to the surgery.

Please note that we only register new patients between 14:00 and 16:00 each day

You will need to collect and complete a registration form from the Surgery together with a patient health questionnaire. If you wish to register your children you will need to complete a registration form and questionnaire for each child.

We will need to see a form of photo ID, for example your passport, driving licence or a bus pass. We will also need to see a proof of address which must have your name and address on it. Examples of a suitable proof of address are a utility bill or a bank statement. We regret we are unable to accept mobile phone bills as proof of address.  You will be required to sign a consent regarding your Data and how you require us to use it. 

Once you have completed your questionnaire, please make an appointment to see our Registration Clerk, and bring in your completed forms and ID with you.
