Please use the link below to access our online appointment and queries service. We will no longer be accepting appointment requests over the phone. This service is only available between 08:00 - 16:00 Monday to Friday or until the Triage reaches full capacity. If you require an urgent appointment between 16:00 and 18:30 you will need to ring 01494 474783. Please book Nurse and HCA appointments via the phone. When submitting your form you will then received a text message or phone call from the receptionist to advise when your appointment has been scheduled for. You may be asked further questions to ensure your queries is dealt with correctly. We are hoping this service allows our patients more access and reduces wait times on the phone. If you are unable to access the form or unable to complete it please call reception and they will complete the form for you online. Receptionists are unable to book on the day appointments with a GP or ANP via the telephone.

For more information, please contact reception or next time you visit the surgery, bring with you a piece of photo ID (passport, driving licence) and something with your address on like a bank statement but not a gas or electric bill. If you don’t have these, please talk to us and we will help you sign-up

GP online services allow you to access a range of services via your computer, mobile or tablet. Once you have signed up, you will be able to:

• book or cancel appointments online
• renew or order repeat prescriptions online
• view parts of your GP record, including information about medication, allergies, vaccinations, previous illnesses and test results

You can still contact the practice by phone or in person, this is just another option, which other patients have found is more convenient and saves them time. To be able to access appointments for results you will need online access, please use the links below. Forms will not be accepted if not signed and sent in with the correct ID.

ONLINE ACCESS - PROXY

Proxy access form needs to be completed if you would like someone else to have access to your records or you would like to access someone elses records. If this is for a child over the age of 11 they will need to sign the form to confirm they are happy for you to have access to this. At 16 your access will be stopped automatically and will need to be re confirmed that the child is still happy for you to have access to their records.
